Does your child have a medical condition/allergies/other concerns?

It is very important that parents/guardians inform the school of any medical concerns immediately.  Please be sure to do the following: 

1. Complete the Atlanta Public Schools - VHE Clinic Form

2. Inform the school's nurse. 

3. Inform your child's homeroom teacher
